The care provided is individualized and evaluated by the Executive Director and staff on a continual basis. Resident service plans evolve as a resident’s needs fluctuate.

MONTHLY PRICING INCLUDES:

• All utilities, except phone and cable

• Three nutritious and appealing meals

• Daily menu and special diet accommodations

• Light weekly housekeeping and laundry service

• Maintenance and grounds keeping

• 24/7 on site caregivers

• Routine rooms checks and safety monitoring

• Ongoing comprehensive assessments

• Social and recreational activities

• Care coordination if applicable (PT, OT, etc.)

• Personalized service plan

• Full medication administration*

• Basic assistance with ALL activities of daily living

* No medications will be allowed in the resident’s room – over the counter or prescribed. This includes medicated creams, lotions, etc.

• Bathing

• Dressing

• Toileting

• Ambulation

• Oral Care

• Assistance with Hearing Aids

• Assistance with Medical Equipment (oxygen, nebulizer, etc.)
